Office Policies

In order to be fair to all of our guests, if you need to reschedule your appointment it must be done at least 2 days before so we have time to fill the appointment time with another guest. Since Dr. Widen does not double-book appointments and hop around from room to room it is vital that you call our office no less than 48 hours before any appointment you can not make. This dedicated time reserved specifically for you is one of the many things that sets our office apart from the rest of the dental community. We reserve the right to charge you for the time that was reserved for you if you miss an appointment, reschedule or cancel your appointment without sufficient time notice

All services must be paid for at time of service. Most dental insurance forms will be sent electronically.  TMJ treatment requires a narrative, superbill and other documentation that we will email to you to attach to a medical claim form or uploaded to a claim portal.

For your convenience all forms will be securely emailed to you to complete online and submit securely back to us. They come from our secure Hushmail server.  If you do not see them please check your spam folder.
